Botox® Has Range: Surprising Lesser Known Uses for the Wrinkle Treatment by Dr. Anil Shah

Botox is practically synonymous with line smoothing, but it was originally used to treat abnormal eye movements – and physicians soon realized that the same mechanism could be useful far beyond the face. Botox is a neuromodulator and, while its name has become widely used as shorthand for the category, it is actually one of several neuromodulator brands available today.
Botox's FDA-approved uses now extend well beyond aesthetics, from chronic migraine and excessive sweating to muscle spasticity and overactive bladder, while doctors also use it off-label for conditions including jaw clenching, facial asymmetry and certain nerve-related movement disorders.
Botox Has Always Been a Multitasker
Botox works by temporarily blocking the release of acetylcholine, the chemical signal that tells muscles to contract. That can soften a wrinkle, but the same action can also calm an overactive muscle, reduce glandular activity or interrupt abnormal movement.
Its FDA-approved therapeutic uses include chronic migraine, underarm hyperhidrosis (excessive sweating), cervical dystonia, overactive bladder and eye muscle conditions including strabismus and blepharospasm.
Some of Its Best Uses Aren’t About Appearance
Jaw clenching and teeth grinding are two examples. Injecting Botox into the masseter and temporalis muscles can reduce excessive activity and ease tension. For people with hyperhidrosis, it can help control sweating when other treatments aren’t enough. And for those with chronic migraine, Botox is FDA-approved to help prevent headaches.
Lesser-known applications include intradermal Botox for oily skin and enlarged pores, as well as its use for facial asymmetry and synkinesis following facial nerve injury. Patients often think of Botox as something that stops a muscle from moving because they don’t want a wrinkle. But the same principle can be used when a muscle is moving too much, a gland is producing too much or a movement pattern has become dysfunctional.
Off-Label Is Not the Same as Experimental
Many of these lesser-known applications, including TMJ and bruxism, facial asymmetry, pore reduction, oily skin and some facial movement disorders, are considered off-label. That simply means they have not received a specific FDA indication for that use. Off-label prescribing is common in medicine and allows physicians to apply a drug’s established mechanism beyond its approved indications.
What matters is understanding the distinction. Patients should understand the difference between an FDA-approved use and something a physician is using based on clinical judgment and available evidence.
